CPBC invites all BC pharmacy professionals to participate in the Safety Attitudes Questionnaire (SAQ) survey.
This evidence-informed survey was developed by the Institute for Safe Medication Practices Canada (ISMP Canada) in collaboration with the Accessing Centre for Expertise (ACE) and an expert advisory panel to assess safety culture in pharmacy practice.
Your participation will provide us with valuable insights into safety culture within pharmacy practice environments across BC.

Why This Matters
As pharmacy professionals working directly with patients, your perspective is vital to advancing our strategic goal of continuous quality improvement.
The SAQ supports CPBC’s commitment to enhance public care and safety by identifying opportunities to learn and improve pharmacy practice and health outcomes. This survey is being conducted as CPBC’s new medication incident reporting (MIR) regulatory framework becomes effective in all BC pharmacies after a one-year transition period ending June 1, 2026. MIR will contribute to improved patient safety by establishing a framework for documenting medication incidents, analyzing their root causes, and sharing de-identified learning across Canada.
The feedback you provide will allow us to establish a baseline understanding of current safety culture in BC pharmacies which will help to ensure a successful implementation of MIR.

Confidentiality
Your responses are completely anonymous and confidential. The survey is administered by ISMP Canada, and only aggregate data will be shared with CPBC.
