About CIRCL

CIRCL is CPBC's comprehensive medication incident reporting program for all community and hospital pharmacies in BC. It's a framework designed to enhance medication safety through structured reporting, analysis, and shared learning.

Medication incidents are a leading cause of preventable harm in healthcare systems worldwide, and in BC, medication errors represent some of the most common complaints received by CPBC.

CIRCL provides a structured approach to quality improvement that helps pharmacies identify and address risks before incidents occur, learn from incidents systematically, enhance team collaboration, and create safer practice environments for patients and staff.

Transition Period: As of June 1, 2026, all CIRCL requirements will be in effect.

Key requirements include:
  • Having access to a compliant reporting platform that integrates with national databases (NIDR for community pharmacies, NSIR for hospital pharmacies),
  • Documenting all medication incidents with required information,
  • Holding annual routine team meetings, and
  • Completing safety self-assessments.
Additional Resources

CPBC is continuing to develop comprehensive resources to support pharmacies and pharmacy professionals through the transition to CIRCL implementation, including updated FAQs, and implementation guides.

These resources will be released progressively during the transition period.
