This session centres the human impact of medication incidents and explores how safety culture, learning, and patient  partnership  work together to support safer care in pharmacy practice.

About this Webinar

This webinar will be hosted by ISMP Canada and led by Melissa Sheldrick, ISMP Canada’s first Patient and Family Advisor. Melissa brings a powerful lived perspective to medication safety advocacy following the loss of her son, Andrew, due to a dispensing error involving a compounded medication.

Since 2019, Melissa has worked nationally and internationally to advance medication safety by sharing her experience and calling for meaningful system-level change. Her work reminds us that behind every prescription is a person—and that learning and accountability are essential to medication safety.

Melissa is joined by her colleague, Enna Aujla, Director of Community Pharmacy Reporting and Learning Programs at ISMP Canada who will share her experience as a pharmacist in connecting CIRCL to practice.

This is the third installment in CPBC’s four-part CIRCL webinar series, designed to support pharmacies throughout the transition period.

This session builds on that foundation by connecting the technical requirements of CIRCL to their real-world purpose: safer care for patients and families. The final session in the series will continue to support pharmacies as they move toward full implementation by June 1, 2026.

About CIRCL

CIRCL is CPBC's comprehensive medication incident reporting program for all community and hospital pharmacies in BC. It's a framework designed to enhance medication safety through structured reporting, analysis, and shared learning.

Medication incidents are a leading cause of preventable harm in healthcare systems worldwide, and in BC, medication errors represent some of the most common complaints received by CPBC.

CIRCL provides a structured approach to quality improvement that helps pharmacies identify and address risks before incidents occur, learn from incidents systematically, enhance team collaboration, and create safer practice environments for patients and staff.

Transition Period: As of June 1, 2026, all CIRCL requirements will be in effect.

Key requirements include:
  • Having access to a compliant reporting platform that integrates with national databases (NIDR for community pharmacies, NSIR for hospital pharmacies),
  • Documenting all medication incidents with required information,
  • Holding annual routine team meetings, and
  • Completing safety self-assessments.

CPBC has developed a Quick-Start Checklist to help pharmacies begin operationalizing CIRCL requirements.

The checklist is intended to be an easy-to-use reference that you can view online or print and keep on hand as you work through the transition to CIRCL. It walks you through the key steps you’ll need to take, including confirming that your incident reporting platform meets CIRCL requirements, establishing policies and procedures, training your team, holding regular team meetings, and completing safety self-assessments.
