Team Meetings

Regular team meetings are essential for maintaining a focus on quality improvement and patient safety. These meetings provide a structured opportunity for the pharmacy team to discuss safety concerns, review incidents and near misses, and collaborate on solutions.

According to CPBC's requirements:

  • Initial meeting deadline: An initial team meeting must be held before June 1, 2027, for pharmacies operating as of the June 1, 2026 effective date (one year after the transition period ends).
  • Ongoing frequency: After the initial meeting, further team meetings must be held at least once a year.
  • Documentation: Each meeting must be documented, and include the following details:
    • Date of the meeting
    • Names of pharmacy staff members in attendance
    • Topics of discussion
    • Any resulting improvement plans

As an example, these meetings should include:

  Review of medication incidents and near misses that have occurred

  Discussion of contributing factors and root causes

  Evaluation of existing action plans and their effectiveness

  Review of relevant information from external sources (such as ISMP bulletins)

  Celebration of safety successes and improvements

  • These requirements can also be met by incorporating the required content and documentation into the pharmacy’s existing operational team meetings
  • Team meetings can be great places to discuss or complete the safety self-assessment. Most software tools also allow you to complete the safety self-assessment in sections that can be saved and completed in one or more meetings.
