According to the Health Professions Act Bylaws Schedule F, pharmacy professionals must:
- Disclose the incident to the patient or patient's agent in accordance with a patient-centered approach appropriate to the patient's needs
- Follow up with the patient or patient's agent to monitor for effects of the incident as appropriate
- Share information about the incident and follow-up plan with other health professionals involved in the patient's circle of care as appropriate
- Communicate improvements with the patient or patient's agent about how the pharmacy will improve and share learnings to prevent recurrence, when appropriate
Pharmacy managers should ensure their policies and procedures provide clear guidance to staff about how to approach disclosure conversations effectively.
