UBC’s Faculty of Pharmaceutical Sciences invites recently graduated pharmacists (2007-2013) to a one-day Interprofessional Medication Reconciliation Program on Monday, January 12, 2015.

The $200 registration fee will be reimbursed upon completion of program and submission of program evaluation form. The program is also eligible for CE credits.
Each location will host practicing pharmacists as they join over 550 3rd year pharmacy, 4th year medicine and nursing, and Nurse Practitioner students to demonstrate collaborative, responsive and responsible communication with other healthcare practitioners.
Pharmacists will also will also learn how to understand and successfully perform medication reconciliation, and apply the knowledge and clinical skills necessary to recognize, evaluate and resolve medication discrepancies at transitions of care to enhance patient safety.
After completing the program, you will also be able to identify evidence-based information and employ communication technology effectively to optimize patient health outcomes, and explain the rationale for physicians, pharmacists, nurses and nurse practitioners to work together for benefit of their patients.
The program will take place at the University of Victoria, the University of British Columbia – Okanagan, and the University of Northern BC from 9:00 a.m. - 12:00 p.m.
