Name Changes

If any of the following scenarios applies to you, email the Name Change application form with the applicable documentation to the Professional Licensing team at [email protected] or by fax at: 604-733-2493 or 1-800-377-8129:

  • Change in your surname following marriage
  • Change in your surname back to your maiden name
  • Change in your legal name
  • Change in your informal name
  • Reporting a new informal name or an informal name that is not currently on your record

You will receive a confirmation email once your record has been updated.
