The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Diamondali Alladin Tejani to suspend his methadone dispensing privileges for a 30 day period commencing July 16, 2012 and continuing until August 15, 2012. During this time Mr. Tejani may practice pharmacy, however he is prohibited from participating in any activity which in any way relates to the dispensing of methadone to patients. The Inquiry Committee considers the agreement necessary to protect the public.

- Nature of Action: of the College of Pharmacists of British Columbia (the “College”) investigated a complaint about the conduct of Hajra Mirza (the “Registrant”), pursuant to section 33(1) of the Health Professions Act, RSBC 1996, c 183 (“HPA”).
The Inquiry Committee and the Registrant agreed to resolve all matters arising from the investigation by way of a Letter of Undertaking under section 36(1) of the HPA.

- Admissions and Acknowledgements:
The Registrant admitted and/or acknowledged the following facts:- While employed as a pharmacist at a pharmacy, the Registrant prepared a fraudulent employment document under the letterhead and purported signature of her current employer and submitted this fraudulent document to a prospective employer;
- She was untruthful during her employer’s subsequent investigation into the document; and
- She remained untruthful during the College’s investigation of her employer’s complaint about the document until she was warned about obstructing a College inspector.
- While employed as a pharmacist at a pharmacy, the Registrant prepared a fraudulent employment document under the letterhead and purported signature of her current employer and submitted this fraudulent document to a prospective employer;
- Disposition:
The Registrant consented to a Letter of Undertaking, wherein the Registrant consented to terms that included the following:
- To be suspended as a registrant of the College for a period of 30 days from June 25, 2012 to July 25, 2012;
- To pay a fine in the amount of $3000.00;
- To provide a copy of the Inquiry Committee’s disposition to her current employer;
- To complete the Code of Ethics Educational Tutorial and submit a signed Declaration of Completion (with Pharmacy Oath) associated with the Code of Ethics Educational Tutorial; and
- To meet with the Deputy Registrar to discuss her conduct in this matter and the legal and ethical standards required of College registrants.
- To be suspended as a registrant of the College for a period of 30 days from June 25, 2012 to July 25, 2012;
- Rationale:
The Inquiry Committee considered that although this was the Registrant’s first disciplinary matter, the deliberateness of the Registrant’s conduct and the potential for serious harm to patients, together with her disturbing pattern of repeated dishonesty and untrustworthy behaviour that only came to an end when provided with unequivocal documentary proof, warranted significant disciplinary action.The Inquiry Committee wished to deliver a clear message to all registrants that a period of suspension will result when registrants act in a dishonest, misleading or unresponsive manner during the course of a complaint investigation by College inspectors.
