Change of Authorized Representative (AR)

CHANGE OF AUTHORIZED REPRESENTATIVE (AR)

This information is applicable to a direct owner that is a hospital, an association, a society, a university or the government.  For a direct owner that is a corporation, sole proprietor (pharmacist) or partnership of pharmacists, refer to the Change of Direct Owner (for sole proprietor or partnership of pharmacists) or Change of Indirect Owner page (for corporation(s)) for steps to notify the College of the change in the authorized representative.

An authorized representative (AR) of the direct owner is responsible for:

  • Applying for a new pharmacy licence;
  • Renewing or reinstating a pharmacy licence; and/or
  • Notifying the College of changes to previously submitted licensure information.

If the direct owner of your pharmacy is a hospital, an association, a society, a university or the government, one or more individuals were assigned by the Direct Owner as their authorized representative(s). 

Note: that the current pharmacy manager will be notified of the change application.

  1. Apply for a Change of Authorized Representative

    When an individual is departing from his/her role as an authorized representative and/or when an individual has been appointed to become an authorized representative of the direct owner, the current authorized representative must submit a Change of Authorized Representative Application as soon as possible before the change occurs. The new authorized representative is not required to complete Proof of Eligibility

    Allow up to 7 days for processing. 

    The authorized representative who submitted the application will receive an email confirmation upon completion of the change application. No changes to the pharmacy licence will be made for this type of change. 

    Note: This change process and requirements are specific to changes of authorized representatives only. Should other changes accompany this change (e.g. Change of Manager), a seperate application for that particular change is required.
