The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Ms. Hajra Mirza to suspend her registration as a pharmacist for a 30 day period commencing June 25, 2012 and ending on July 25, 2012. The Inquiry Committee considers the agreement necessary to protect the public.

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Jason William Keller to place limits and/or conditions on his registration as a pharmacist effective June 16, 2012. The agreement remains in effect until June 15, 2014. The Inquiry Committee considers the agreement necessary to protect the public.

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Shun Fu (Ron) Kan to suspend his registration as a pharmacist commencing June 12, 2012 and continuing until further notice. The Inquiry Committee considers the agreement necessary to protect the public.

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Benton M. Lee to suspend his registration as a pharmacist for a 90 day period commencing May 14, 2012 until Monday, August 12, 2012. The Inquiry Committee considers the agreement necessary to protect the public. 

Omara, Ayman Sayed Mohammed (May 12, 2012)

Ayman Omara has satisified the terms of an agreement reached with the Inquiry Committee dated May 12, 2010. Limits and conditions on Mr. Omara's registration as a pharmacist have now been removed.

The Inquiry Committee has reinstated pharmacist registrant’s registration and pursuant to Section 36 of the Health Professions Act, has reached an agreement with pharmacist registrant to place limits and/or conditions on her registration as a pharmacist effective December 20, 2011. The agreement remains in effect until further notice. The Inquiry Committee considers the agreement necessary to protect the public. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act.

The Inquiry Committee has reinstated pharmacist registrant’s registration and pursuant to Section 36 of the Health Professions Act, has reached an agreement with pharmacist registrant to place limits and/or conditions on his registration as a pharmacist effective September 25, 2011. The agreement remains in effect until further notice. The Inquiry Committee considers the agreement necessary to protect the public. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act.

Sanchez, Isidoro (Andres Rudy)/Marigold Compounding and Natural Pharmacy (Sep 16, 2011)

The Inquiry Committee has reached an agreement with lsidoro (Andres Rudy) Sanchez and Marigold Compounding and Natural Pharmacy pursuant to section 36 of the Health Professions Act. lsidoro (Andres Rudy) Sanchez and Marigold Compounding and Natural Pharmacy have agreed to comply with all regulatory requirements, effective September 16, 2011. This agreement will remain in effect until further notice. The lnquiry Committee considers the agreement necessary to protect the public.


(SEPTEMBER 11, 2011 - Registration reinstated)

The Inquiry Committee, pursuant to Section 35 of the Health Professions Act, has suspended Isidoro (Andres Rudy) Sanchez’s registration as a pharmacist effective immediately pending completion of an investigation. The Inquiry Committee considers this action necessary to protect the public. 

Grant , Derek James (Aug 26, 2011)

Derek James Grant has satisfied the terms of an agreement reached with the Inquiry Committee dated August 26, 2010. Limits and conditions on Mr. Grant’s registration as a pharmacist have now been removed.
