Professional Development and Assessment Program Evaluation and Feedback Form In 2003, the College of Pharmacists initiated the Professional Development and Assessment Program (PDAP). So far, more than 1200 pharmacists have participated in the program.As one of the participating pharmacists, we would like to hear about your experience with PDAP in Phase 2 or program evaluation. What worked well for you? What could have been better? Did the program fulfill its purpose? Use this feedback form to provide your thoughts and observations in a quick and systematic way. Please complete the form only once, either online or on paper. It should only take you between 10 to 15 minutes to complete the form. If you would like to offer additional comments, you are welcome to do so at the end of the form. You do not need to provide any identifying information. Your input is extremely important, not just to the College but to all BC pharmacists. The feedback you provide will enable us to continue to refine and improve the program, making it as responsive as possible to the needs of BC pharmacists. This is particularly important as we continue through the second cycle of PDAP and beyond.
