Change of Director

  1. Apply for Change of Director

    Submit all the required documentation to the College (i.e. application form and Form 10 – Notice of Change of Directors (obtained from your legal counsel) signed by the owner/director prior to the effective change date.

    Please allow up to 5 business days for processing.

    Note: Incomplete applications may cause delays and will not be processed until all documentation/requirements have been met. If application has not been approved, the College will contact the owner/director.

  2. College Notification to PharmaCare Upon approval of the application, the College will notify PharmaCare.
  3. Completion of Director Acceptance Letter
    Letter(s) will be forwarded to the pharmacy director(s) for completion. Completed copy(ies) must be returned to the College via email: or fax: 604.733.2493 within 5 business days.
  4. Pharmacy Licence
    The new pharmacy licence is accessible through eServices.

The pharmacy director(s) have now been changed.
